CCM High Performance Boys 15 Program Registration

The CCM Minnesota/USA Hockey High Performance programs are designed to identify the best players in Minnesota and to provide the top players from each district or section a chance to play with and against the best players in the state.

Players must reside (residence) in the Minnesota District of USA Hockey to participate in the CCM High Performance Player Development Programs. Players who participate on Minnesota District (high school or youth) teams but reside in another state must participate in their home state's player identification process.

This registration will enable you to provide contact information, sign consents and submit payment. Please read through each page carefully to ensure that all information is provided accurately.
